ConnectEd Birthday Party Guest Waiver

Parent or Legal Guardian: Please complete this form for each child who will be participating in a ConnectEd birthday party.

This waiver must be completed before your child participates in ConnectEd activities.

Activities at ConnectEd

ConnectEd provides supervised technology, gaming, creative, and educational activities.

Depending on the party, activities may include computer gaming, Minecraft, multiplayer games, digital art, animation, music production, video production, coding, and other computer-based activities.

Activities may involve the use of computers, monitors, keyboards, mice, headphones, controllers, drawing tablets, furniture, and other electronic or creative equipment.

Acknowledgment of Risk

I understand that participation in recreational and technology-based activities involves ordinary risks.

These may include accidental injury, collisions with furniture or equipment, falls, discomfort associated with prolonged computer use, minor accidents involving equipment or personal belongings, and other ordinary risks associated with participating in activities at a computer and creative technology facility.

I understand that ConnectEd takes reasonable measures to provide a safe environment and supervise activities, but that not every risk can be eliminated.

Release and Waiver

To the fullest extent permitted by applicable California law, I voluntarily assume the ordinary risks associated with my child's participation in ConnectEd activities and agree to release and hold harmless ConnectEd Tech & Gaming LLC, its owners, employees, instructors, and agents from claims arising from my child's participation to the extent such claims are legally waivable.

This waiver does not waive or release any claim or right that cannot legally be waived under California law.

Rules & Conduct

I understand that my child must follow ConnectEd's rules and staff instructions.

I understand that ConnectEd may restrict or end my child's participation if their behavior creates a safety concern, damages property, significantly disrupts an event, or violates ConnectEd rules.

I understand that I may be responsible for damage to ConnectEd property caused by my child beyond ordinary wear and tear.

Emergency Information

I understand that ConnectEd staff are not medical professionals and that ConnectEd does not provide medical care.

In the event of an emergency, I authorize ConnectEd staff to contact emergency services when reasonably necessary.

I understand that ConnectEd will make reasonable efforts to contact me or the emergency contact listed above.

Personal Belongings

I understand that my child is responsible for their personal belongings.

ConnectEd is not responsible for lost, stolen, or damaged personal property brought into the facility.

Parent/Guardian Agreement

By submitting this form, I confirm that:

  • I am the parent or legal guardian of the participant named above.

  • I have read and understand this Participant Liability Waiver.

  • I understand the general nature of the activities my child may participate in.

  • I understand and voluntarily accept the ordinary risks associated with participation.

  • I agree that my child will follow ConnectEd's rules and staff instructions.

  • I understand that ConnectEd may restrict or end my child's participation for safety or behavioral reasons.

  • I understand that this waiver applies to my child's participation in ConnectEd events and activities.
